Individuals who feel they have experienced discrimination may file a signed, written complaint within one hundred eighty (180) days from the date of the alleged discrimination.
The complaint should include the following information:
Your name, mailing address, and contact information
How, when, where, and why you believe you were discriminated against. Include the location, names and contact information of any witnesses.
Other information such as pictures, witness information or other media if appropriate
