Birth & Death Certificates

Vital Records Fee

All  birth and death certificates are $25 for the first record and $5 for each additional record if purchased in the same transaction. If you are not sure a record is available or ready to be released, you can pay an $25 search fee for our clerks to search for the record.

Birth Certificates

The State of Georgia Vital Records office has established a websites for Local Custodians in which a person may request a birth certificate from any county in Georgia. Most new births are accessible within 3 to 5 days, and births from previous years as well. (In rare cases, due to minor problems with the system, this office will not be able to access your birth record, but in this instance you may still contact your birth county.) 

Requests

When requesting a certified copy, you will need:

  1. A valid photo ID
  2. Supplying the full name at birth
  3. Birth date
  4. Birth place (county)
  5. Father’s full name
  6. Mother’s full maiden name
  7. Relationship to same, which must be either 
    • Adult sibling
    • Father
    • Grandparent
    • Mother
    • Self
  8. The number of copies

If a guardian, be prepared to show certified proof of same.

If requesting by mail, we need the aforementioned information as well as a copy of your valid photo ID, your signature witnessed by a Notary Public, and money order payable to Probate Court, along with your return address.

Please note that a father who is not listed on the birth record and stepparents will not be authorized to receive a copy.

Death Certificates

Only direct family members may request a copy of a death certificate for individuals who either died in Douglas County or who lived in Douglas County but passed away in another location; however, the cause of death will be omitted for anyone not directly related to the decedent. The only exceptions are insurance companies/law firms that need documentation for a case.

Requests

When requesting a certified copy, you will need 

  1. A valid photo ID
  2. As well as supplying the full name of deceased
  3. Date of death
  4. Your relationship to decedent
  5. The number of copies

If requesting by mail, we need the aforementioned information as well as a copy of your valid photo ID, your signature witnessed by a Notary Public, and money order payable to Probate Court, along with your return address.
